6. Motivation (Describe your personal interest for learning about skin diseases in SOC and diversity groups and how you apply your knowledge and skills)

8. Reach (Describe whether your project has local, regional, national, or global reach, and if it is scalable for future projects)
9. Milestones, Timeline and Budget (Please take into consideration feasibility for these parameters)

Please provide a summary of your innovative project and motivation for applying. Mentorship Plan (limit 1,500 characters, or approximately 250 words)
Identify the mentor’s name who will oversee the project and rationale for their selection. Project Timeline (October 2025 - October 2026 - limit 1,500 characters, or approximately 250 words)
Outline the proposed timeline and key deliverables for the innovative project including identified touchpoints with mentor on progress of the initiative, timing of the 1-year summary share-back report at the 12th Skin Spectrum Summit annual conference. Budget Accountability (limit 1,500 characters, or approximately 250 words)
Briefly describe how you plan to use the $20,000 scholarship for your proposed innovative project. Please include any travel related to your innovative project, with the exception of travel to the Skin Spectrum Summit conference (this will be covered separately). Note that purchased equipment cannot be kept at the end of the project and can be donated. A budget reconciliation will be required at the end of your project.
